UnitedHealthOne Copay Select Value plan allows two office visits per person per year (subject to a $35 copay). Generic prescription coverage is included and subject to a $15 copay. Adult and children preventative doctor office visits are also included but no copay applies. Mammograms, pap smears and PSA testing are also covered with no cost to policyholders . A wide range of deductibles is available, making this plan one of the most affordable Ohio low cost health insurance policies.
Anthem SmartSense Plus Standard RX plan is a slight upgrade from the Anthem Value plan (which is no longer available). Three (instead of two) office visits are covered, subject to a $35 copay. Subsequent office visits are subject to a deductible, although a Network discount may apply. Formulary prescriptions are subject to the greater of a $15 copay or 40% . Non-formulary drugs receive the Anthem negotiated discount. Adult and child preventative coverage is included and the annual lifetime maximum cap has been removed due to national health care reform.
Celtic Basic plan provides two office visits that are subject to a $30 copay. The deductible and coinsurance applies to subsequent office visits. Adult and preventive benefits are covered "first dollar" while generic and name-brand prescription coverage is included subject to a $1,000 deductible.
Aetna Preventive and Hospital Care 3000 plan is perhaps the ultimate “low cost" plan. Premiums continue to be low, despite a rate increase in mid-2009. Essentially, the plan is a catastrophic policy that also includes comprehensive preventive coverages. Annual routine GYN exams, Pap/mammogram and routine physicals are all covered without having to meet the $3,000 policy deductible. The annual lifetime maximum of $1 million has been removed. Aetna Ohio medical plans have been growing in popularity since 2005.
UnitedHealthOne Copay Select plan offers the most coverage of any of the low cost plans. Unlimited covered office visits are subject to a $35 copay and most preventative benefits are included. Generic and non-generic prescriptions are included along with a dental/vision discount program included in the premium. However, to keep premiums low, a major medical deductible of at least $2,500 should be selected. In many areas of Ohio (especially the Central and Southwestern part), this health insurance plan is extremely popular.
